An antral follicle count, or AFC, measures the number of small follicles visible in the ovaries during an ultrasound. A high AFC generally indicates that a relatively large number of follicles are visible at the time of evaluation and can provide useful information about ovarian reserve and expected response to fertility medications.

A high antral follicle count can be normal for some patients and may also be seen in people with polycystic ovarian morphology or PCOS. Your Spring Fertility physician will interpret AFC alongside AMH, age, menstrual history and other clinical information rather than using the follicle count alone.

What Does a High Antral Follicle Count Mean?

Antral follicle count, or AFC, is the number of small follicles visible in the ovaries during a transvaginal ultrasound, typically near the beginning of a menstrual cycle. These follicles represent the group from which eggs may potentially develop during that cycle.

A high AFC generally indicates a relatively large number of recruitable follicles and can be associated with a higher ovarian reserve. Like AMH, however, AFC primarily provides information about egg quantity rather than egg quality.

What Can Cause a High Antral Follicle Count?

A high AFC may be associated with:

  • Higher ovarian reserve: Some patients naturally have a larger pool of visible antral follicles.
  • Younger reproductive age: Follicle counts generally decrease as ovarian reserve declines with age.
  • Polycystic ovarian morphology: A larger number of small follicles may be seen in people with PCOS, although AFC alone does not diagnose the condition.
  • Individual variation: Follicle counts can vary between patients and somewhat between cycles.

Your physician interprets AFC alongside AMH, age, menstrual history and other findings to understand what the result means for fertility and potential treatment response.

High AFC and PCOS

A high antral follicle count can sometimes be seen in patients with polycystic ovarian morphology or polycystic ovary syndrome (PCOS). In these cases, ultrasound may show a larger number of small follicles within the ovaries.

However, a high AFC does not by itself mean that you have PCOS. PCOS is a clinical diagnosis that considers multiple features, which may include irregular or absent ovulation, signs or laboratory evidence of elevated androgens and characteristic ovarian findings.

This distinction matters because someone can have a high ovarian reserve without having PCOS, while patients with PCOS can have different symptoms, hormone patterns and fertility needs. Your Spring Fertility physician can interpret AFC alongside menstrual history, AMH, hormone testing and other clinical findings to determine whether the follicle count has broader significance.

When fertility treatment is needed, a high AFC can also help anticipate ovarian response. Patients with many recruitable follicles may respond strongly to stimulation medications, so medication dosing and monitoring can be individualized to pursue an appropriate response while reducing treatment-related risks.

What Happens After a High Antral Follicle Count?

A high antral follicle count is generally interpreted alongside AMH, age, menstrual history and other clinical findings. If your cycles are regular and there are no other concerning findings, a relatively high AFC may simply indicate a larger ovarian reserve.

If a high follicle count occurs alongside irregular or absent ovulation, elevated androgen levels or other characteristic findings, your physician may consider whether evaluation for PCOS is appropriate. A high AFC by itself does not establish the diagnosis.

For IVF or egg freezing, AFC can also help your physician anticipate how the ovaries may respond to stimulation medications. Patients with a large number of recruitable follicles may require individualized medication dosing and careful monitoring as follicles develop.

In many cases, the finding does not require treatment itself but provides useful information for designing a safe and individualized fertility treatment plan.
