Semen parameters can vary between samples, and one abnormal measurement does not necessarily establish a diagnosis or determine which fertility treatment you will need. What Does an Abnormal Semen Analysis Mean?

A semen analysis evaluates several characteristics of semen and sperm. A result may be considered abnormal when one or more measurements fall outside the laboratory’s reference range.

An abnormal result does not necessarily mean that pregnancy cannot occur naturally. Semen parameters can vary between samples, and the significance of a finding depends on which parameters are affected, how significantly they differ from reference values and whether other fertility factors are present.

Which Semen Analysis Results Can Be Abnormal?

A semen analysis typically evaluates multiple measurements rather than producing a single fertility score. These can include:

  • Semen volume: The amount of fluid in the ejaculate.
  • Sperm concentration: The number of sperm present per unit of semen.
  • Total sperm count: The estimated total number of sperm in the sample.
  • Motility: The percentage of sperm that are moving and characteristics of their movement.
  • Morphology: The percentage of sperm meeting specific criteria for shape and structure.
  • Total motile sperm count: An estimate of the total number of moving sperm available in the sample.

Because these measurements interact, your physician considers the complete semen analysis rather than interpreting one number in isolation.

What Can Affect Semen Analysis Results?

Semen analysis results can vary for many reasons, and an abnormal finding does not always indicate a permanent problem with sperm production. The period of abstinence before collection, whether the complete sample was collected, recent illness and other temporary factors can influence results.

When abnormalities persist, your physician may consider a broader range of potential contributors. Hormonal factors can affect sperm production, while conditions involving the testes or reproductive tract may affect sperm quantity or delivery. Varicocele, certain medications or medical treatments, genetic factors and other health conditions may also be relevant depending on the pattern and severity of the results.

The specific semen parameters affected can help guide the evaluation. A low sperm concentration raises different questions than isolated poor morphology, while low semen volume combined with no sperm may warrant a different evaluation than reduced motility alone.

When indicated, additional evaluation may include reproductive hormone testing, genetic testing or referral for specialized male reproductive evaluation. What Happens After an Abnormal Semen Analysis?

The next step after an abnormal semen analysis depends on which parameters are affected and the degree of the abnormality. Because semen measurements can vary, your physician may recommend a repeat semen analysis to determine whether the finding persists before drawing conclusions from a single sample.

If abnormalities are confirmed, additional evaluation may help identify contributing factors. Depending on the results and medical history, this may include reproductive hormone testing, genetic testing or specialized evaluation of male reproductive health. Findings such as azoospermia or a severely reduced sperm count generally require a different evaluation than an isolated abnormality in morphology or motility.

The complete fertility picture also matters when considering treatment. Sperm concentration, motility, morphology, semen volume and total motile sperm count can be considered alongside factors involving the other reproductive partner, including age, ovarian reserve, ovulation and fallopian tube function.
