Fertility is not solely determined by eggs, ovulation, or the uterus. Sperm-related factors can also play an important role in whether conception occurs, and they may be present alone or alongside other reproductive factors. A semen analysis is often an important part of a fertility evaluation because it provides information about sperm count, movement, shape, and other characteristics. What Is Male Factor Infertility?

Male factor infertility is a broad term for sperm or other male reproductive factors that may reduce the likelihood of conception. These factors can affect the production, quantity, movement, shape, function, or delivery of sperm.

Male factor infertility can occur on its own or alongside female reproductive factors. Because conception depends on both egg and sperm when using a male partner’s sperm, evaluating sperm health can be an important part of understanding why pregnancy has not occurred.

What Can Cause Male Factor Infertility?

Male fertility can be affected by many different biological, medical, genetic, anatomical, and environmental factors. In some cases, a specific cause can be identified. In others, semen analysis may show differences in sperm parameters without revealing a clear underlying explanation.

Factors that may contribute to male infertility include:

  • Low sperm count
  • Reduced sperm motility
  • Differences in sperm morphology
  • Problems with sperm production
  • Blockages or other reproductive tract conditions that affect sperm delivery
  • Varicocele
  • Certain hormonal or genetic conditions
  • Previous infections, surgeries, or medical treatments
  • Certain medications or exposures that can affect sperm production
  • Multiple contributing factors

A fertility evaluation can help determine which findings are relevant and whether additional assessment may be appropriate.

How Is Male Fertility Evaluated?

A semen analysis is one of the primary tests used to evaluate male fertility. It examines several characteristics of semen and sperm that can influence the likelihood of fertilization.

Depending on the results and medical history, evaluation may consider:

  • Semen volume
  • Sperm concentration and total sperm count
  • Sperm motility, or how sperm move
  • Sperm morphology, or sperm shape
  • Other characteristics of the semen sample

Sperm parameters can vary between samples, so an abnormal result does not necessarily provide a complete diagnosis on its own. Repeat testing or additional evaluation may sometimes be recommended. When appropriate, a reproductive endocrinologist may also recommend evaluation by a reproductive urologist or another specialist.

What Do Semen Analysis Results Mean?

Semen analysis results are interpreted together rather than relying on a single number. For example, a lower sperm count does not necessarily mean that fertilization cannot occur, and differences in morphology or motility must be considered alongside the other characteristics of the sample and the broader reproductive picture.

The significance of a semen analysis also depends on factors involving the egg source, including age and ovarian reserve, as well as how long pregnancy has been attempted, previous pregnancies or fertility treatment, and family-building goals.

For this reason, semen analysis is best understood as one component of a comprehensive fertility evaluation rather than a definitive measure of whether someone can or cannot conceive.

How Is Male Factor Infertility Treated?

The appropriate approach depends on the type and severity of the sperm-related factor, whether an underlying condition can be identified, and the reproductive circumstances of both partners or gamete sources.

Depending on the findings, options may include addressing an underlying medical or reproductive condition when appropriate, continued attempts to conceive, intrauterine insemination (IUI), in vitro fertilization (IVF), IVF with intracytoplasmic sperm injection (ICSI), or other individualized approaches.

Some male reproductive conditions may benefit from evaluation or treatment by a reproductive urologist. In other circumstances, fertility treatment may be used to help overcome a sperm-related barrier to fertilization rather than directly treating the underlying cause.

When Might IVF or ICSI Be Considered?

IVF may be considered when sperm-related factors significantly reduce the likelihood of fertilization through intercourse or IUI, when other fertility factors are also present, or when previous treatment has not resulted in pregnancy.

During conventional IVF, eggs and sperm are placed together in the laboratory to allow fertilization to occur. With intracytoplasmic sperm injection (ICSI), an embryologist selects an individual sperm and injects it directly into an egg. ICSI may be recommended in certain cases of male factor infertility or based on previous fertilization history and other clinical considerations.
