What Does Poor Embryo Quality Mean?
During IVF, embryologists evaluate embryos as they develop in the laboratory. Embryo quality generally refers to observable characteristics of development, including the embryo’s stage, appearance and cellular organization at the time it is assessed.
An embryo described as lower quality or receiving a lower morphological grade has characteristics that differ from those associated with more favorable embryo grades. However, embryo grading is not a definitive measure of reproductive potential. Lower-graded embryos may still be capable of implantation and pregnancy, while a highly graded embryo does not guarantee a successful outcome.
How Is Embryo Quality Evaluated?
Embryologists assess different characteristics depending on the embryo’s stage of development. At the blastocyst stage, grading commonly considers how expanded the blastocyst is and the appearance of the inner cell mass and trophectoderm.
Morphological grading describes what an embryo looks like at a particular point in development. It is different from chromosome testing. When PGT-A is performed, the embryo’s chromosome result provides a separate type of information from its morphological grade, and both may be considered when evaluating embryos available for transfer.
What Can Embryo Quality Tell Us About an IVF Cycle?
Embryo quality is most useful when considered as part of the complete pattern of embryo development during an IVF cycle. Your physician and embryology team can review how many eggs were retrieved and mature, how many fertilized normally, how embryos progressed through development and how many ultimately reached the blastocyst stage.
The pattern across the embryo cohort can provide more context than the grade assigned to any single embryo. For example, one lower-graded embryo among several developing blastocysts presents a different picture from a cycle in which multiple embryos show slower or poorer development.
Egg factors, sperm factors and normal biological variation can all influence embryo development, and there is not always a single identifiable reason why an embryo receives a particular grade. Embryo appearance also cannot reveal every characteristic relevant to reproductive potential.

What Happens After Poor Embryo Quality?
After embryos receive lower grades or development is poorer than anticipated, your physician and embryology team can review the complete IVF cycle rather than relying on embryo grade alone. This includes ovarian response, egg maturity, fertilization, the pattern and timing of embryo development and the number of embryos that reached the blastocyst stage.
Embryo morphology provides useful information, but it does not perfectly predict reproductive potential or chromosome status. If PGT-A was performed, genetic testing results provide a different type of information from morphological grading and can be considered separately when making decisions about available embryos.
If another IVF cycle is being considered, the previous cycle may provide information that helps your physician evaluate whether any aspect of stimulation, fertilization or treatment planning warrants reconsideration. However, embryo development is influenced by complex biology, and there is not always an identifiable or modifiable reason for lower embryo quality.
