What AMH Can—and Cannot—Tell You

AMH helps estimate ovarian reserve, but it does not measure fertility or predict whether you will become pregnant naturally.

Anti-Müllerian hormone (AMH) reflects the number of developing follicles within the ovaries and helps physicians estimate how many eggs may respond during an egg freezing cycle.

However, AMH has important limitations. It does not measure egg quality, determine whether someone is infertile, or predict exactly when menopause will occur.

For these reasons, fertility specialists interpret AMH alongside other clinical findings rather than in isolation.

Understanding Ovarian Reserve

Ovarian reserve refers to the number of eggs remaining in the ovaries—not the quality of those eggs.

Every person is born with a finite number of eggs, and that supply naturally declines over time.

Estimating ovarian reserve helps fertility specialists predict how the ovaries may respond to stimulation medications and whether fertility preservation should be considered sooner rather than later.

A lower ovarian reserve does not necessarily mean pregnancy is impossible, and a higher ovarian reserve does not guarantee future fertility.

Why Age Still Matters

Age remains one of the strongest predictors of egg quality.

Even when ovarian reserve appears normal, egg quality gradually changes over time. This is one reason fertility specialists continue to consider age alongside laboratory testing.

Two individuals with similar AMH levels may receive different recommendations because their ages—and therefore the expected quality of their eggs—are different.

What Ultrasound Adds

Ultrasound provides important information that laboratory tests alone cannot.

During a fertility evaluation, physicians often perform an ultrasound to measure the antral follicle count (AFC), which estimates the number of small resting follicles within the ovaries.

Ultrasound also allows physicians to evaluate ovarian anatomy and identify findings that may influence treatment planning.

When interpreted together, ultrasound and AMH provide a more complete assessment of ovarian reserve.

Why Doctors Rarely Focus on One Number

One laboratory value rarely tells the whole story.

For example:

  • A lower-than-expected AMH may be less concerning in a younger patient.
  • A normal AMH does not guarantee fertility will remain stable for years.
  • Ultrasound findings may support—or differ from—laboratory results.
  • Medical history often changes how laboratory values are interpreted.

Rather than asking whether one result is “good” or “bad,” fertility specialists ask what the complete pattern suggests.

What Does “Normal” Really Mean?

Normal fertility testing does not necessarily mean fertility will remain unchanged in the future.

Many patients are reassured when their fertility evaluation is normal, and appropriately so. However, reproductive aging continues over time.

Similarly, a result outside the typical range does not automatically mean pregnancy will be difficult or that egg freezing is urgently needed.

“Normal” is best understood as one piece of a broader clinical picture.

When Results May Change Recommendations

Sometimes fertility testing changes the conversation.

For example, physicians may recommend discussing egg freezing sooner if testing suggests lower ovarian reserve than expected for age.

In other cases, reassuring fertility testing may support waiting, monitoring fertility over time, or pursuing pregnancy naturally when the patient is ready.

The purpose of testing is to personalize recommendations—not to direct everyone toward treatment.

What Happens If My Results Are Unexpected?

Unexpected fertility test results can be surprising, but they rarely provide the entire answer on their own.

Your physician may recommend repeating certain tests, obtaining additional laboratory work, or performing further evaluation before making treatment recommendations.

Unexpected findings often lead to a more detailed conversation—not an immediate conclusion.

Can Fertility Testing Predict My Future?

No. Fertility testing estimates probability—it does not predict certainty.

No physician can determine exactly when fertility will decline or whether pregnancy will occur naturally.

Instead, fertility testing helps estimate reproductive potential based on current information. Those estimates become more meaningful when combined with your age, medical history, and future family-building goals.

Putting the Whole Picture Together

Fertility specialists rarely ask whether one laboratory value is normal.

Instead, they ask:

  • What does this patient’s age tell us?
  • What does the ovarian reserve appear to be?
  • Are the laboratory and ultrasound findings consistent?
  • Does the medical history change our interpretation?
  • When does the patient hope to become pregnant?
  • Is egg freezing likely to provide meaningful future benefit?

Only after considering all of these factors together do physicians make individualized recommendations.
