What Is Ovarian Reserve Testing?
Ovarian reserve testing provides information about the number of eggs remaining in the ovaries and how the ovaries may respond to fertility medications. It is commonly used during fertility evaluation, IVF planning and egg freezing consultations.
Ovarian reserve is primarily a measure of egg quantity, not egg quality. Age remains an important factor associated with egg quality and the likelihood that an egg will result in a chromosomally normal embryo. For this reason, ovarian reserve results should be interpreted alongside age and the rest of the fertility evaluation.
What Does Ovarian Reserve Testing Tell You?
Ovarian reserve testing can help a fertility specialist estimate expected response to ovarian stimulation and individualize treatment planning. It may also identify findings consistent with a lower or higher ovarian reserve than expected for a patient’s age.
No ovarian reserve test can determine whether someone can or cannot become pregnant naturally. A lower AMH or antral follicle count does not mean pregnancy is impossible, and a higher result does not guarantee fertility. These tests answer a narrower clinical question about the remaining follicle pool and expected ovarian response.
Which Tests Evaluate Ovarian Reserve?
Ovarian reserve is typically evaluated using more than one piece of information. Two of the most useful measures are AMH and antral follicle count, with other hormone testing sometimes providing additional context.
AMH Testing
Anti-Müllerian hormone (AMH) is produced by cells surrounding developing follicles in the ovaries. Because AMH can provide information about the remaining follicle pool, it is commonly used to assess ovarian reserve and help anticipate response to ovarian stimulation.
Antral Follicle Count
An antral follicle count (AFC) is performed using transvaginal ultrasound. A fertility specialist counts the small follicles visible in the ovaries early in the menstrual cycle. AFC provides another measure of ovarian reserve and can help inform expectations for treatment response.
FSH and Estradiol
Early-cycle FSH and estradiol may also be considered when evaluating ovarian function. Because hormone values can vary and influence one another, they are interpreted alongside AMH, AFC, age and the broader clinical picture rather than in isolation.
These measures provide complementary information. A fertility specialist can determine which findings are most relevant for an individual patient and how they should influence treatment planning.

What Does Low Ovarian Reserve Mean?
Lower AMH or antral follicle count may indicate a smaller remaining follicle pool and can be associated with a lower expected response to ovarian stimulation. These findings do not mean that pregnancy cannot occur and do not directly measure egg quality.
What Does High Ovarian Reserve Mean?
Higher AMH or antral follicle count generally indicates a larger follicle pool and may be associated with a stronger response to ovarian stimulation. Higher values are sometimes seen in patients with polycystic ovary syndrome (PCOS), but an elevated result alone does not diagnose PCOS.
Age remains particularly important because ovarian reserve testing primarily provides information about egg quantity, while age is more closely associated with changes in egg quality and chromosome status.
