Failed implantation can occur when an embryo does not successfully establish a pregnancy after transfer or attempted conception. Because implantation depends on multiple biological factors, a single unsuccessful implantation does not necessarily indicate that something is wrong.

Why Can Embryo Implantation Fail?
Implantation depends on multiple biological steps occurring successfully. The embryo must have the developmental potential to continue growing, the uterine environment must be able to support implantation and the timing of embryo development and endometrial preparation must align appropriately.
When reviewing an unsuccessful embryo transfer, your fertility specialist may consider several areas:
- Embryo factors: Embryos differ in their developmental potential, and embryo chromosome abnormalities can affect the ability to implant and continue developing.
- Uterine factors: The anatomy and environment of the uterine cavity may be considered when evaluating implantation.
- Endometrial preparation: Your physician may review how the uterine lining developed and how medications were used during a fresh or medicated embryo-transfer cycle.
- Transfer history: Details from the embryo-transfer procedure and previous transfers can provide additional clinical context.
- Reproductive history: Previous pregnancies, pregnancy losses and fertility treatment can help your physician understand whether an unsuccessful transfer appears to be an isolated event or part of a broader pattern.
Sometimes, however, an unsuccessful embryo transfer occurs despite favorable conditions and no single identifiable explanation.
When Is Additional Testing Considered After Failed Implantation?
One unsuccessful embryo transfer does not necessarily mean that extensive additional testing is needed. Even embryos with favorable characteristics do not always implant, and an unsuccessful transfer can occur without an identifiable problem with the uterus, transfer procedure or treatment protocol.
Your physician may consider additional evaluation when your broader history suggests there is a specific question worth investigating—for example, after multiple unsuccessful transfers, when previous imaging raises a uterine concern or when another reproductive or medical factor may be relevant.
Depending on your circumstances, your fertility specialist may review or recommend evaluation of the uterine cavity, previous embryo development, available embryo genetic information, endometrial preparation and other aspects of prior treatment. Testing should be selected because it can answer a clinically relevant question rather than simply because an embryo transfer was unsuccessful.
What Are the Next Steps After Failed Implantation?
The appropriate next step depends on what your cycle and reproductive history show. For some patients, another embryo transfer using a similar protocol may be reasonable. For others, the previous cycle may provide a reason to modify endometrial preparation, investigate the uterine cavity, reconsider embryo-related factors or make another individualized adjustment.
If additional embryos are available, your physician can discuss what is known about those embryos and how that information affects expectations for another transfer. If another IVF cycle is needed, information from previous egg retrievals, fertilization and embryo development can also help inform the treatment strategy.
Importantly, adding more tests, medications or procedures is not automatically better. The goal is to determine whether there is evidence supporting a change and focus treatment on factors that are relevant to your particular history.
