Missed Periods and Fertility

A missed period occurs when an expected menstrual period does not happen. Pregnancy is one possible explanation, but missed periods can also result from changes in ovulation, stress, weight, exercise, medications, hormones, or other health factors.

What Is a Missed Period?

A missed period occurs when an expected menstrual period does not happen. Pregnancy is one possible explanation, but periods can also be missed when ovulation is delayed or does not occur.

What Causes Missed Periods?

Possible causes include pregnancy, PCOS, stress, changes in weight or energy availability, intensive exercise, thyroid or prolactin disorders, perimenopause, primary ovarian insufficiency, medications, and other hormonal factors.

Repeated missed periods may indicate that ovulation is occurring infrequently or not at all. When ovulation does not occur regularly, opportunities for conception are reduced, although many causes of ovulatory dysfunction can be treated.

When Missed Periods Should Be Evaluated

Pregnancy should be considered when appropriate. Repeatedly missed periods, prolonged absence of menstruation, or missed periods accompanied by other hormonal symptoms should also be evaluated, particularly when pregnancy is desired. A longer absence of menstruation may be considered amenorrhea.

Evaluation may include pregnancy testing, menstrual and medical history, hormone testing, and pelvic ultrasound. When fertility is a concern, physicians may also assess ovarian reserve and whether ovulation is occurring.
