Understanding Low Testosterone and Male Fertility

What Does a Low Testosterone Result Mean?

Testosterone is an important reproductive hormone involved in sexual function and sperm production. A low blood testosterone level may indicate that hormone production or hormonal signaling is not functioning as expected.

Low testosterone does not automatically mean that sperm production is impaired, and testosterone levels alone cannot determine male fertility. Semen analysis provides more direct information about sperm production.

What Can Cause Low Testosterone?

Potential contributors can include:

  • Testicular dysfunction: The testes may not produce testosterone normally.
  • Pituitary or hypothalamic factors: Hormonal signals that stimulate testosterone production may be reduced.

If fertility is a goal, it is particularly important to discuss testosterone treatment with a fertility physician because therapies used to increase testosterone levels can have very different effects on sperm production.

Can Testosterone Therapy Affect Fertility?

Yes. External testosterone can suppress the hormonal signals needed for sperm production. When testosterone is introduced from outside the body, the brain and pituitary gland may reduce signaling through hormones such as LH and FSH, which can decrease the testosterone concentration within the testes that is necessary for normal sperm production.

As a result, some patients taking testosterone may develop a substantially reduced sperm count or even have no sperm detected in the ejaculate. This is why fertility goals are important to discuss before starting testosterone replacement therapy, particularly for someone who wants to conceive now or preserve the possibility of having biological children in the future.

If you are already using testosterone and are trying to conceive, do not make medication changes without discussing them with a physician. The appropriate approach depends on why testosterone was prescribed, your hormone levels, semen analysis findings and reproductive goals.
