FSH and estradiol are among the hormones commonly associated with Day 3 testing. Depending on the clinical situation, other bloodwork or an ultrasound may also be performed as part of an early-cycle fertility evaluation.

Does Testing Have to Happen Exactly on Day 3?

The appropriate testing window depends on the specific test and your physician’s instructions. Some early-cycle assessments may be performed within a range of cycle days rather than exclusively on Day 3. Which Hormones Are Tested on Day 3?

Estradiol is produced by developing ovarian follicles. Measuring estradiol alongside FSH can provide important context because an elevated early-cycle estradiol level may affect how an FSH result is interpreted.

Other Testing

Depending on the patient, other bloodwork or an ultrasound may be performed around the same time. Modern ovarian reserve evaluation often incorporates AMH and antral follicle count in addition to traditional Day 3 hormone testing.

How Are Day 3 Test Results Interpreted?

Day 3 hormone results are interpreted in relation to one another and alongside other measures of ovarian reserve. A single FSH or estradiol result cannot independently determine fertility or predict whether pregnancy will occur.

FSH reflects signaling from the pituitary gland to the ovaries, while estradiol is produced by developing follicles. An elevated early-cycle estradiol level can affect FSH, which is one reason the values may be reviewed together rather than independently.
