What Is a Fertility Evaluation?

A fertility evaluation is a comprehensive assessment designed to identify factors that may be affecting conception and help determine appropriate next steps. It typically combines a detailed medical and reproductive history with testing selected for the individual patient or couple.

The evaluation may consider ovulation, ovarian reserve, the uterus and fallopian tubes, sperm-related factors and relevant hormonal or genetic considerations. Not every patient needs every test. The appropriate workup depends on age, reproductive history, how long pregnancy has been attempted, previous pregnancies or fertility treatment and any known reproductive conditions.

Why a Complete Evaluation Matters

Conception depends on several reproductive processes working together. A single normal test therefore cannot rule out every potential fertility factor, just as one abnormal result does not necessarily explain why pregnancy has not occurred.

By reviewing results together, a fertility specialist can identify which findings are clinically meaningful, whether additional testing is warranted and which treatment options may be appropriate.

What Happens During a Fertility Evaluation?

A fertility evaluation usually begins with a detailed conversation about reproductive and medical history. Your physician may ask about menstrual cycles, previous pregnancies, how long pregnancy has been attempted, prior fertility treatment, surgeries, medications and known reproductive conditions.

Initial Consultation

The consultation helps your fertility specialist understand your goals and identify which parts of the reproductive process should be evaluated. If you already have fertility test results or records from previous treatment, these may also provide useful information.

Diagnostic Testing

Testing may include ovarian reserve assessment, hormone bloodwork, ultrasound and evaluation of the uterus or fallopian tubes. When sperm is part of the reproductive plan, semen analysis is often an important component of the evaluation.

Reviewing the Complete Picture

Once testing is complete, your physician considers the findings together rather than treating each result as a separate diagnosis. This integrated review can help identify potential fertility factors, determine whether additional testing is appropriate and guide a personalized discussion about next steps.

What Fertility Tests May Be Recommended?

The tests recommended during a fertility evaluation depend on which parts of the reproductive process need to be assessed. Your physician may combine several evaluations to develop a more complete picture.

Testing for Ovarian Function and Ovulation

Bloodwork and ultrasound may be used to assess ovarian reserve, reproductive hormones and ovulation. Common measures include AMH, antral follicle count, FSH, estradiol and, when clinically relevant, progesterone, LH, thyroid function or prolactin.

Uterine and Fallopian Tube Evaluation

Ultrasound can provide information about the ovaries and uterus, while tests such as an HSG can evaluate whether the fallopian tubes appear open. Saline sonography or hysteroscopy may be considered when more detailed evaluation of the uterine cavity is needed.

Sperm Evaluation

When sperm is part of the reproductive plan, semen analysis evaluates sperm concentration, motility, morphology and other characteristics. Additional male fertility testing may be recommended when semen analysis or medical history suggests that further evaluation could be useful.
